The Outpatient Nurse Coordinator is a Registered Nurse is a member of the care delivery team who is responsible for the care of the patient in a global view, accountable for multiple aspects of the patient care package even when patient contact is minimized. The Outpatient Nurse Coordinator uses independent judgment and continued communication with the patient, family, physician and community resources to assist the patient in attaining a measure of outpatient health. Responsibilities:

Key Responsibilities

  • Provides comprehensive, patient-centered nursing care across the continuum of care.
  • Coordinates and manages care plans, ensuring individualized treatment and follow-up for assigned patients.
  • Serves as the primary nursing resource and advocate for patients and families.
  • Triages patient concerns and collaborates with providers to ensure timely, appropriate care.
  • Assists providers with patient assessment, treatment, education, and care management.
  • Coordinates care with internal departments, home health agencies, and community resources.
  • Maintains accurate, timely, and comprehensive patient documentation.
  • Monitors clinic operations, patient flow, and daily workflow to promote efficient care delivery.
  • Ensures compliance with regulatory, accreditation, and organizational standards.
  • Participates in quality improvement initiatives aimed at enhancing patient outcomes, safety, and satisfaction.
  • Identifies and addresses patient care, safety, and operational improvement opportunities.
  • Promotes a positive, collaborative practice environment through effective communication and teamwork.

Leadership & Professional Practice

  • Provides clinical leadership and guidance to nursing staff, support staff, and new employees.
  • Supports staff onboarding, training, competency validation, and professional development.
  • Serves as a mentor, preceptor, and resource for colleagues and learners.
  • Participates in shared governance, evidence-based practice, and professional development activities.
  • Assists leadership with departmental planning, operational initiatives, and process improvements.
  • Fosters a culture of accountability, continuous learning, and patient advocacy.

  • Must be able to demonstrate the knowledge and skills necessary to provide care and/or interact appropriately to the ages of the patients served.
  • Must also demonstrate knowledge of the principles of growth and development over the life span and possess the ability to assess data reflective of the patients status and interpret the appropriate information needed to identify each patients requirements relative to his/her age-specific needs and to provide the care needs as described in the department policy and procedures.
  • Minimum 3 years nursing experience.
  • BSN preferred.
  • Previous Outpatient experience preferred

  • Previous pediatric expereince preferred.
    Licensure, Certifications, and Clearances:
     
  • CPR Certification
  • UPMC approved national certification preferred.
  • CPR required based on AHA standards that include both a didactic and skills demonstration component within 30 days of hire
  • Basic Life Support (BLS) OR Cardiopulmonary Resuscitation (CPR)
  • Registered Nurse (RN)*
  • Act 33 / 34 / 73 with renewal

*Current licensure either in the state where the facility is located or, if the facility is in a state covered by the multistate Nursing Licensure Compact (NLC) agreement, a multistate license issued by a participating NLC state. Hires and current employees working on an out-of-state NLC license who later change their residency to the state where the facility is also located will have 60 days upon changing their residency to apply for licensure within that state.
