In this Aging Case Aide 2 you will primarily work with our Intake/Assessment Unit to help contact seniors across Luzerne and Wyoming counties and connect them with services they need. You may also support Social Services Department staff in our other Units as needed to help the whole team provide responsive and high-quality services. Your work will include: 

  • Outreach and Follow-Up: Contact consumers by telephone to help with screening referrals for assessment and following up on emergency situations, unmet needs, or any other reasons as necessary
  • Record Keeping: Document communication from calls in our database accurately and consistently
  • Incoming Calls: Answer incoming telephone calls form consumers who have questions about Aging programs
  • Application Support: Help consumers complete request forms and gather documentation
  • Public Contact: Conduct home visits to consumers who need more help gathering financial documentation or other application information; pickup and deliver consumer necessities including emergency medical supplies, emergency food, or paperwork
  • Activity Support: Track participation; assist volunteers and staff with providing senior center meals, ensuring food sanitation, and planning, conducting, and encouraging participation in senior center community activities

Work Schedule and Additional Information:

  • Full-time employment
  • Work hours are 8:00 AM to 4:00 PM, Monday - Friday, with a 30-minute lunch.
  • Telework: You will not have the option to telework in this position.
  • Salary: In some cases, the starting salary may be non-negotiable.

QUALIFICATIONS

Minimum Experience and Training Requirements:

  • One year as an Aging Case Aide 1; or 
  • Two years of public contact work, including one year in a human services program; or 
  • Any equivalent combination of experience and training.

Other Requirements:

  • You must be able to perform essential job functions.
