This Aging Care Manager 3 position with our Community Based Services Division, Assessment Unit strengthens the safety net for elderly residents who need expert support. This position performs advanced complex case management and referral functions, offers services to help eliminate risk, and works with consumers, families, and community partners ensure consumer well-being. As an Aging Care Manager 3, you will perform the following duties:
- Review Referrals: Evaluate medical, social, psychological, and other factors for consumers referred for assessment, including heavy advanced assessments on critical cases
- Complete Assessments: Performs on-site, face to face assessments of homebound, frail, elderly consumers in hospitals, care facilities, and their own homes in the community and remote areas; discuss and explain services with consumers and families
- Determine Risk: Identify risk, including in-depth abuse factors or eminent danger of life and health, and coordinate next steps with supervisors and protective services, expediting cases when needed
- Audit Cases: Complete intense and detailed chart and desk reviews checking for completeness, adherence to criteria, and comparison to other cases; prepare for, attend, and testify at appeal hearings
- Coordinate Communication: Serve as a liaison for DHS/AAA with social service agencies, medical facilities, and community organizations; answer calls, emails, and questions from consumers, families, and facility staff
- Support Team Development: Provide training, assistance, and mentoring to other caseworkers in the unit; interact with outside agencies and providers for outreach, training, and education

Work Schedule and Additional Information:
- Full-time employment
- Work hours are 8:30 AM to 4:30 PM, Monday - Friday, with a 60-minute lunch.
- Telework: You may have the opportunity to work from home (telework) part-time, up to two days per week, after six months of employment. In order to telework, you must have a securely configured high-speed internet connection and work from an approved location inside Pennsylvania. If you are unable to telework, you will have the option to report to the headquarters office in Pittsburgh. The ability to telework is subject to change at any time. QUALIFICATIONS
Minimum Experience and Training Requirements:
- One year as an Aging Care Manager 2 or Aging Program Assessor; or
- One and one-half years of professional human services, law enforcement, or nursing experience; and a bachelor's degree in social sciences, behavioral sciences, human services, or a closely related field; or
- An equivalent combination of experience and training which includes six months of professional human services, law enforcement, or nursing experience.
