Join our Inpatient Rehabilitation team as an Intake Coordinator, where you will play a vital role in coordinating patient admissions and ensuring a seamless transition into rehabilitation services. In this fast-paced position, you will serve as a key liaison between patients, families, physicians, referral sources, and clinical staff, helping to facilitate timely admissions and maintain exceptional patient experiences.
What You'll Do:
Coordinate and process inpatient rehabilitation referrals and admissions.
Communicate with referral sources, healthcare providers, patients, and families to gather information and facilitate care transitions.
Verify required documentation, insurance authorization, and admission criteria.
Collaborate closely with interdisciplinary teams to ensure efficient patient placement and admission workflows.
Maintain accurate patient records and monitor referral status throughout the intake process.
Provide outstanding customer service while supporting patients and families during their rehabilitation journey.

Experience in healthcare administration, patient access, scheduling, admissions, case management support, or a related field is preferred.

Purpose:
Provide essential support to the Inpatient Rehabilitation Team by managing communication between patients, staff, providers, schedulers, and all UPMC departments involved in patient care. Receives and processes clinical information pertinent to the facilitation of prior authorizations for medications. Facilitates the comprehensive continuity of care of the inpatient rehabilitation patient, including, but not limited to the transplant, stroke, and LVAD population.
Responsibilities:
- Coordinates and assists medical management of the inpatient rehabilitation patient including but not limited to patient communications, outside physician offices, scheduling all required appointments and testing, obtaining authorizations for medications, coordinating with outpatient pharmacies, collaborating with Rehab Clinicians and Post-Acute Liaison for home care and equipment needs, and clerical support.
- Demonstrate the ability to establish priorities, perform assignments and meet productivity thresholds in the allotted time frames while maintaining accuracy and attention to detail. Maintain patient confidentiality. Maintain weekly/monthly productivity. Support Rehab Clinicians and members of the Inpatient Rehabilitation Team.
High school diploma or equivalent required. Associate degree preferred. Two years' experience with administrative support in an outpatient clinic, physician office or other health care setting. Knowledge of medical terminology required. Ability to handle a high-volume workload and to remain focused and organized under pressure. Ability to multi-task and meet deadlines. Ability to perform duties with significant degree of independence and initiative. Strong interpersonal, communication, and customer service skills required. Proficiency in Microsoft Office products preferred.
