Aging Care Manager 2 (Local Government) - Wayne County Area Agency on Aging

In this role, you will support older adults by completing assessments, arranging services, and responding to needs in a variety of settings. You work is essential in promoting independence and ensuring consumers receive the right services at the right time. As an Aging Care Manager 2, you will perform the following duties:

  • Intake Support: Gather information from consumers and provide referrals and explanations of available services
  • Service Coordination: Arrange and monitor services for consumers with simple to complex needs
  • Assessment Work: Conduct initial and annual assessments in homes, facilities, hospitals, or by phone
  • Record Management: Document case activity, complete reports, and maintain accurate files
  • Community Collaboration: Work with families, providers, and community agencies to support consumer needs
  • Travel Requirements: Travel within the county to meet consumers in varied locations and conditions

Work Schedule and Additional Information:

  • Full-time employment
  • Work hours are 8:00 AM to 4:30 PM, Monday - Friday, with a 60-minute lunch.
  • On-call hours when needed. Overnight travel or out of county travel may be needed
  • Telework: You will not have the option to telework in this position. 
  • Salary: In some cases, the starting salary may be non-negotiable.

QUALIFICATIONS
 
Minimum Experience and Training Requirements:

  • Six months as an Aging Care Manager 1 or a County Caseworker 1; or
  • Successful completion of the County Social Casework Intern program; or
  • Six months of professional human services or nursing experience; and a bachelor’s degree in social sciences, behavioral sciences, human services, or a closely related field; or
  • An equivalent combination of experience and training.

Other Requirements:

  • You must be able to perform essential job functions.

Legal Requirements: 

  • You must pass a background investigation.
  • A conditional offer of employment will require a drug screening.
  • This position falls under the provisions of the Child Protective Services Law.
    • Under the Law, a conditional offer of employment will require submission and approval of satisfactory criminal history reports including, but not limited to, PA State Police clearance, PA Child Abuse history clearance, and FBI Fingerprint clearance.
