This position performs public contact work that assists elderly consumers and their family members with accessing a wide range of services and programs through our agency and other community resources. It helps them make informed choices to meet present needs and decide on future long-term plans in the context of each person’s needs, preferences, values, and individual circumstances. As an Aging Case Aide 2, you will perform the following duties:
- Referral Tracking: Use computer systems to enter data, review, and determine the status of new and existing consumer referrals
- Intake Processing: Interview consumers to identify their needs, strengths, and eligibility for aging related services; provide clear explanations of available programs and community resources; make referrals as appropriate
- Person-Centered Counseling: Help consumers weigh pros and cons, then decide on an action plan that meets their needs; provide assistance with completing applications and accessing options
- Case Follow-up: Contact consumers, informal supports, referral sources, and other community agents to ensure consumer needs are met
- Documentation: Maintain an ongoing record of all contacts made, detailed case histories, and resolutions; prepare memos, letters, summaries, and reports
- Community Engagement: Participate in community events to support consumers and in-service training opportunities to increase knowledge, technical expertise, and personal growth
Work Schedule and Additional Information:
- Full-time employment
- Work hours are 8:30 AM to 4:30 PM, Monday - Friday, with a 60-minute lunch.
- Telework: You will not have the option to telework in this position.
- Salary: In some cases, the starting salary may be non-negotiable.

QUALIFICATIONS
Minimum Experience and Training Requirements:
- One year as an Aging Case Aide 1; or
- Two years of public contact work, including one year in a human services program; or
- Any equivalent combination of experience and training.
Other Requirements:

- You must be able to perform essential job functions.
